CLINICAL
Care Coordinator/Transitional Care Nurse - Part to full-time, days - The Transitional Care Nurse (TCN)/Care Coordinator will travel to local hospitals to provide evaluations and meet with our patients to provide them options for care after hospitalization. TCN will be cross-trained in all liaison team roles and an RN degree is preferred (must be licensed with the NH Board of Nursing). The role requires strong clinical skills, excellent organizational and communication skills with the ability to work in a fast-paced environment. Wound Care Specialist (RN) - Homecare - Part to Full-time - 24-40 hours per week position. The CWCN/WOCN will primarily consult with and educate clinical staff and further developing the Agency's wound care program. Position requires a BSN and RN license from NH Board of Nursing. It is preferable that the RN have home care experience and Oasis-C knowledge but we will train the right candidate. Advanced knowledge of wound care treatment plans,excellent communication, organization and presentation skills are a must. The RN will become an integral part of the homecare team - salaried position.

MANAGEMENT AND ADMINISTRATIVE
Development Assistant - Part-time - The development assistant will report to the Director of Development and be primarily responsible for activities surrounding the donor software database. Recent Raiser's Edge program experience is required. The assistant will also support the department with donor events and he/she must be computer savvy and have strong communication skills, both written and verbal.
CLIENT SUPPORT SERVICES
Homemakers - Weekdays and some weekends - -Flexible part-time or full-time hours available helping the elderly and disabled remain at home
-Perform light housekeeping, help prepare meals, run errands and provide transportation to medical appointments
-Clients in greater Manchester and Greater Nashua
-Seeking candidates with excellent people skills and a genuine commitment to helping others
-High school diploma or GED, reliable transportation, and auto insurance required
