CLINICAL
APRN or Physician - Hospice - Part-time - Days, with on-call - We currently have an opportunity for a part-time Nurse Practitioner or Physician to join our hospice team and work in collaboration with the hospice team members in delivering end-of-life services. The practitioner or physician will work closely with our hospice and board certified medical directors with patients at our Community Hospice House in Merrimack and will also be responsible for making home visits, evaluating patients and attending weekly interdisciplinary team meetings. The practitioner or physician must be licensed in NH and have end-of-life experience. He/she will also be included in the on-call rotation, which includes weeknights and about one weekend per month. IV RN - Homecare - Part-time - Part-time - An IV certified RN is needed part-time hours on the weekdays for IV homecare visits and admissions, other routine homecare visits and to take IV on-call weekend rotation one or two weekends per month. RN must be licensed by the MA and NH board of nursing and homecare experience is highly preferred. Hours can be flexible during the weekdays and on-call hours include Friday evening to Monday morning.
RN - Homecare - Full-time - Full-time - For Manchester/Bedford area. RN - Homecare - Per diem - Varied - Flexible hours on weekdays and weekends. Social Worker (MSW) - Homecare - Part-time - Part-time position 20-30 hours per week. Requirements include MSW, experience and availability to attend family meetings occasionally. Team Leader - RN - Homecare - 9:30-5:30 M-F - This is a highly visible position requiring a nurse to oversee the clinician's schedules and ensure timely patient visits. Triage calls from patients, staff, doctor's offices and hospitals. The nurse proactively collaborates with intake and homecare managers to anticipate conflicts. A minimum of 2-5 years of home care experience is required. Excellent communication, computer and organizational skills are essential. BSN and triage experience is highly preferred. Team Leader - RN - Hospice - Days, part-time - RN will be the charge nurse responsible for oversight of RNs and LNAs in our hospice program who service patients in nursing homes and patient's homes primarily in the Merrimack and Manchester areas. RN will also triage patient calls and do admission visits as needed. This is a part-time position on first shift and requires prior supervisory experience; CHPN preferred;should have oncology or end-of-life experience. Weekend Baylor RN - Homecare - Days, part-time - We can offer some different attractive opportunities - work either 8 to 12-hour shifts on Saturday and Sunday (either every weekend or every other weekend). Position is perfect for the RN who needs the weekdays off. IV skills and homecare experience is highly preferred. Wound Care Specialist (RN) - Homecare - Part-time - 24-32 hours per week position. The CWCN/WOCN will primarily consult with and educate clinical staff, further developing the Agency's wound care program. The RN will also be asked to complete visits when necessary. Position requires a BSN and RN license from NH and MA. RN must also have home care experience, Oasis-C knowledge as well as advanced knowledge of wound care treatment plans. Excellent communication, organization and presentation skills are a must. The RN will become an integral part of the homecare team.
REHABILITATION STAFF
Physical Therapists (PT) - Days, full-time with benefits - For Hudson, Londonderry, Greater Manchester areas. Position is 37.5-40 hours per week with minimal weekend rotation. PT must have experience with all age groups and have practiced in outpatient rehab facility or homecare setting. NH and MA professional licensure is required.

PRIVATE DUTY DIVISION (non-benefited positions)
Homemakers - Part-Time - Home Health & Hospice Care not only provides hospice, rehabilitation, and nursing services for patients of all ages, but the agency also offers supportive services to help our clients remain safe and independent in their own homes. As a homemaker you will perform light housekeeping duties, help prepare meals, run errands or provide transportation to medical appointments. Homemakers also assist with daily cleaning needs at Community Hospice House as needed. These positions offer flexible hours and schedules to accommodate your family and life. Caseloads are available in greater Nashua and also in Manchester and surrounding towns. Candidates must have a high school diploma, reliable transportation, and proof of auto insurance. We are seeking candidates with excellent people skills and a genuine commitment to helping others. Licensed Nursing Assistants (LNA) - Part or full-time - Part or full-time - LNAs will perform ADLs, light housekeeping and prescription pick up as primary duties. Typically two hour blocks of time are offered to clients in their own homes. Candidates must have current NH LNA licensure, CPR certification, reliable transportation, and auto insurance. Current need is for LNAs for the overnight shift primarily for clients needing respite care and ADL's.
