CLINICAL
Social Worker (Hospice) - Days, part-time - This position offers flexible scheduling for the right candidate. SW must have counseling experience in medical health care setting and hospice / end-of-life care experience. Bachelor's degree in SW or related field is required, MSW is preferred. Staff RN Homecare - Days - Two full-time day shift positions are available in our Homecare Divison. One position will serve the northern Massachusetts area while the other will service the Londerderry/Litchfield area. Qualified candidates must have one year of nursing experience in acute care. Case management and IV skills are highly preferred.
HHHC will provide comprehensive training with our telehealth equipment and electronic documentation program.
REHABILITATION STAFF
Occupational Therapist (OT) - Days, part-time - A per diem/part-time OT is needed weekdays to travel to patients in the greater Nashua and Derry areas. The position offers great flexibility in hours, generous pay and opportunity to increase to full-time hours to cover planned leaves of absence. Candidates must have a NH and MA professional license with 3-5 years of experience. Homecare experience is preferred. Physical Therapist (PT) - Days, part-time with benefits - Position is 20-32 hours per week with flexible schedule. PT must have experience with all age groups and have practiced in outpatient rehab facility or homecare setting. NH and MA professional licensure is required.

PRIVATE DUTY DIVISION (non-benefited positions)
Homemakers - Part-Time - Home Health & Hospice Care not only provides hospice, rehabilitation, and nursing services for patients of all ages, but the agency also offers supportive services to help our clients remain safe and independent in their own homes. As a homemaker you will perform light housekeeping duties, help prepare meals, run errands or provide transportation to medical appointments. Homemakers also assist with daily cleaning needs at Community Hospice House as needed. These positions offer flexible hours and schedules to accommodate your family and life. Caseloads are available in greater Nashua and also in Manchester and surrounding towns. Candidates must have a high school diploma, reliable transportation, and proof of auto insurance. We are seeking candidates with excellent people skills and a genuine commitment to helping others. Licensed Nursing Assistants - Part-time or full-time hours - HHHC is seeking dynamic LNAs to join our team of professionals. Candidates will be cross-trained to work with clients in our home care, hospice, and private duty divisions (including foot care clinics). Primarily first shift hours are available with every other weekend rotation; some evening hours are also available. LNA’s are eligible for bonus after working certain number of hours. Candidates must have recent experience, either as a private duty LNA in a client’s home, or experience in a facility, as well as current NH LNA licensure, CPR certification, reliable transportation, and auto insurance.
