CLINICAL

LNA Homecare - Full-time - Seasoned LNA with at least one year of experience in one or more of the following settings: hospital, nursing home, or VNA. Candidates must have current NH LNA licensure, CPR certification, reliable transportation, and auto insurance. Weekend rotation is required. May be cross-trained for hospice patients.

LNAs Hospice - Seasoned LNAs with at least one year of experience in one or more of the following settings: hospital, nursing home, or VNA. Candidates must have current NH LNA licensure and CPR certification. Weekend hours only (typically one day and one evening shift); may work every weekend or every other weekend.

Housekeeper - Community Hospice House - Per Diem - Flexible part-time hours every other weekend cleaning rooms and completing other duties, including laundry. Prior environmental services experience preferred. Must have high school diploma or GED. Reliability a must.



REHABILITATION STAFF

Physical Therapists - Part-time - Join our outstanding rehabilitation team! Caseloads are available in the teams that serve Wilton, Mason, Milford, Hollis, Mont Vernon, Amherst and Pepperell. One year of experience is required, as well as both NH and MA licenses. HHHC will allow staff to apply for MA licensure upon hire. Must be CPR certified.


MANAGEMENT AND ADMINISTRATIVE

RN - Staff Development Coordinator - Full-time - This full-time position will be responsible for LNA oversight, coordination of care, and clinical staff education and development. Travel is required to patients’ homes for evaluations and supervisory visits. Prior staff development and LNA supervisory experience is preferred. BSN recommended.

Scheduler/Administrative Assistant - This full-time, benefited position develops the schedules for our LNA staff, provides customer service to our private-duty clients, and is responsible for administrative assistant functions reporting to the Director of Marketing & Communications. Medical office experience is required.


CLIENT SUPPORT SERVICES

Homemakers - Home Health & Hospice Care not only provides hospice, rehabilitation, and nursing services for patients of all ages, but the agency also offers supportive services to help our clients remain safe and independent in their own homes. As a homemaker you will perform light housekeeping duties, help prepare meals, run errands or provide transportation to medical appointments. Homemakers also assist with daily cleaning needs at Community Hospice House as needed. These positions offer flexible hours and schedules to accommodate your family and life. Caseloads are available in greater Nashua and also in Manchester and surrounding towns. Candidates must have a high school diploma, reliable transportation, and proof of auto insurance. We are seeking candidates with excellent people skills and a genuine commitment to helping others.


PRIVATE DUTY DIVISION (non-benefited positions)

Licensed Nursing Assistants - Part-time or full schedule - HHHC is seeking seasoned or new graduates to join our dynamic team of professionals. Candidates will be cross-trained to work with clients in our home care, hospice, and private duty divisions (including foot care clinics). Block time schedules are available on first shift, full or partial days. Candidates must have current NH LNA licensure, CPR certification, reliable transportation, and auto insurance.

Homemakers - Part-time - Perform light housekeeping duties, help prepare meals, help clients run errands, or provide transportation to medical appointments. Homemakers also assist with daily cleaning needs at Community Hospice House as needed. Candidates must have a high school diploma, reliable transportation, and proof of auto insurance. Experience is not required, but a genuine commitment to helping others is essential.
