Home Health & Hospice Care is a member of many of the following organizations and hope you find their sites helpful.

Home Care Association of New Hampshire - The Home Care Association of New Hampshire represents licensed agencies that provide in-home health care and supportive services to individuals living in New Hampshire.  The Association promotes the delivery of responsible, high quality healthcare in the home by offering education and networking opportunities for home care professionals, providing leadership on home care issues, and serving as a home care advocate in the public policy arena.

Hospice Foundation of America - Whether you are looking for general information about hospice, or specific information on the Foundation, you'll find their page informative, interesting, and helpful.  On this page you can learn about hospice care, how to select a hospice, and how to locate a hospice near you. You can learn of programs, read excerpts from publications, order books and videos, and sign up for their annual bereavement teleconference.  They invite you to see their hospice stories page and to send in your own story.

National Hospice and Palliative Care Organization - Founded in 1978, the National Hospice and Palliative Care Organization is the oldest and largest nonprofit, public benefit organization devoted exclusively to hospice care. NHPCO is committed to improving end of life care and expanding access to hospice care with the goal of profoundly enhancing quality of life for people dying in America and their loved ones.

National Association of Home Care - HomeCare Online, the National Association for Home Care's virtual headquarters. NAHC is committed in representing the interests of the home care and hospice community.

New Hampshire Hospice and Palliative Care Organization - The New Hampshire Hospice and Palliative Care Organization exists to benefit the people of New Hampshire. In collaboration with others, NHHPCO advocates for improved access to excellent hospice and palliative care and for related services to meet health care needs of people facing life-threatening conditions and provide support to their families.

Visiting Nurse Associations of America - The Visiting Nurse Associations of America (VNAA) was created in 1983 as the official, national association of freestanding, not-for-profit, community based visiting nurse agencies (VNAs).

New Hampshire Hospital Association - The New Hampshire Hospital Association is a not-for-profit association that represents the Granite State's hospitals and health systems. They are improving health by providing members with advocacy, education, information, and shared services.

Foundation for Healthy Communities - The Foundation for Healthy Communities is a not-for-profit organization with a mission to improve health and health care delivery in New Hampshire. The Foundation is a partnership involving hospitals, physicians, health plans, home care agencies, and other organizations concerned about improving health.

Hospice and Care for the Dying

Center to Advance Palliative Care - The Center to Advance Palliative Care is supported by The Robert Wood Johnson Foundation and directed by Mount Sinai School of Medicine.  Their web site offers detailed and practical information to help an institution establish a palliative care program, covering needs assessment, business planning, reimbursements, delivery models, interdisciplinary teams, program operations, and evaluation.  The site includes case studies, showing how other institutions have developed palliative care programs, as well as a directory of over 350 Palliative Care programs with contact information for networking.

Growth House - This award winning web site is your international gateway to resources for life threatening illness and end-of-life issues. Their primary mission is to improve the quality of   compassionate care for people who are dying through public education about hospice and home care, palliative care, pain management, death with dignity, bereavement, and related end of life topics.

Hospice Net -  provides information and support to patients and families facing life-threatening illnesses.  Hospice Net is an independent, nonprofit organization working exclusively through the Internet.

"The End of Life" -  a year long project on death in America by National Public Radio.  There are audio clips with transcripts and many other resources.

On Our Own Terms: Moyers on Dying - An important, informative website developed as part of Moyers' PBS series.  Bill Moyers goes from the bedsides of the dying to the front lines of a movement to improve end-of-life care in ON OUR OWN TERMS: Moyers on Dying.

NH End of Life Project - Posted on the Growth House web page, this two-year project (begun in July 1996) seeks to understand and improve care for persons in New Hampshire who are nearing the end of life. It is funded by the NH Health Care Transition Fund, and directed by Sarah Goodlin, MD, a geriatrician and Assistant Professor of Medicine at Dartmouth Medical School. Jane Ackerman is the project coordinator.

Kearl's Guide to the Sociology of Death and Dying - Trinity University's Department of Sociology and Thanatology presents this guide to the Sociology of Death and Dying.

Wings - an information web site for caregivers and the bereaved.

Last Acts - a call-to-action campaign designed to improve care at the end of life. Our goals are to  bring end-of-life issues out in the open and to help individuals and organizations pursue the search for better ways to care for the dying.
 

Grief and Bereavement

Compassionate Friends - The mission of The Compassionate Friends is to assist families in the positive resolution of grief following the death of a child and to provide information to help others be supportive.

Crisis, Grief & Healing - Tom Golden's page is meant to be a place men and women can browse to understand and honor the many different paths to heal strong emotions. TOM GOLDEN LCSW of Washington D.C. is an internationally known psychotherapist, author, and speaker on the topic of healing from loss.

GriefNet- an Internet community of persons dealing with grief, death, and major loss. They have almost 50 email support groups and two web sites. Their integrated approach to online grief support was developed to provide help to people working through loss and grief issues of many kinds.

WidowNet - an information and self-help resource for, and by, widows and widowers. Topics covered include grief, bereavement, recovery, and other information helpful to people, of all ages, religious backgrounds, and sexual orientations, who have suffered the death of a spouse or life partner.
 

Nursing & Clinical

American Academy of Hospice and Palliative Medicine - The Academy is the professional organization for physicians specializing in hospice and palliative medicine. Membership is also open to nurses and other healthcare providers who are committed to improving the quality of life of patients and families facing life-threatening or serious conditions.

Hospice & Palliative Nurses Association - convenient access to current information in the areas of hospice and palliative nursing, some sections  available for HPNA members only.  Great links to other sources of hospice and palliative nursing information.

New York Times Health Navigator - An excellent guide to health and medical sites on the Internet, compiled by the Technology Editor of the NY Times.

Aging and the Elderly

USA.gov Senior Citizens' Resources - providing official information and services electronically for seniors from the US government.

Alzheimer's Association - the national organization, whose mission is to eliminate Alzheimer's disease through the advancement of research; to provide and enhance care and support for all affected; and to reduce the risk of dementia through the promotion of brain health.

ElderWeb - ElderWeb is an on-line sourcebook with over 4,000 reviewed links to information about health, financing, housing, aging, and other issues related to the care of the frail elderly.

National Council on Aging - working to improve the lives of older Americans.

National Institute on Aging - The National Institute on Aging (NIA) is one of the Institutes of the National Institutes of Health and leads a broad scientific effort to understand the nature of aging and to extend the healthy, active years of life. In 1974, Congress granted authority to form NIA to provide leadership in aging research, training, health information dissemination, and other programs relevant to aging and older people.

SeniorNet - SeniorNet's mission is to provide older adults education for, and access to, computer technology to enhance their lives and enable them to share their knowledge and wisdom.

The American Cancer Society

The National Cancer Institute -  The National Cancer Institute coordinates the National Cancer Program, which conducts and supports research, training, health information dissemination, and other programs with respect to the cause, diagnosis, prevention, and treatment of cancer, rehabilitation from cancer, and the continuing care of cancer patients and the families of cancer patients.

OncoLink - part of the Abramson Cancer Center of the University of Pennsylvania, OncoLink describes itself as the "web's first cancer resource."
