Home Health & Hospice Care is a not-for-profit Visiting Nurse Association and hospice with offices in Nashua, Manchester, and Merrimack, NH. The organization provides home health services to the elderly, the acute and chronically ill, children, newborns, and the dying in the comfort and security of the individual's home.

Among the programs offered by HHHC are:
  • Home Care: Visiting nurses, physical, speech, and occupational therapists, IV therapists, and licensed nursing assistants who provide home care for patients recovering from illness and hospitalization.
  • Palliative and Hospice Care: Nurses, social workers, licensed nursing assistants, spiritual care providers, and volunteers who care for patients who are dying and their families. Pain management and supportive care are integral components of the hospice program.
  • Community Hospice House: A ten suite home that serves patients who do not have a primary caregiver at home or who require end-of-life medical services that cannot be provided in a home setting.
  • Homemaking: A service designed for low-income elderly and disabled people, it is intended to give clients support to enable them to stay independent in their own homes.
  • Community Clinics: HHHC sponsors Foot Care, Blood Pressure, and Flu Clinics in many different communities for the convenience of area residents.
  • Grief Support Groups: Programs for all age groups, from preschoolers to adults, to help them cope with the death of a loved one.
