Every year on April 16, we observe National Healthcare Decisions Day (NHDD) — a nationwide initiative dedicated to inspiring, educating, and empowering individuals and families to engage in advance care planning.
At its core, NHDD is about one simple, powerful idea: your voice matters — and your healthcare choices should reflect your values.
Advance care planning is the process of thinking through and documenting your healthcare wishes in the event that you’re unable to speak for yourself. This can include:
Choosing a healthcare proxy (someone you trust to make medical decisions on your behalf)
Completing an advance directive or living will
Talking openly with your loved ones about what matters most to you — comfort, dignity, autonomy, and quality of life
Too often, these conversations happen in the middle of a crisis — when emotions are high and time is short. But when you take the time to plan ahead:
You ease the burden on your family
You reduce confusion and conflict during difficult times
You ensure your healthcare aligns with your values and beliefs
Advance care planning is a gift — for yourself and for those you love.
Whether you’re just beginning to think about your wishes or ready to put them in writing, here are a few steps you can take:
Start the conversation – With a loved one, a friend, or a healthcare provider
Choose a healthcare agent – Someone you trust to advocate for your values
Complete your documents – Advance directives are legally recognized in most states
Review and update regularly – Your preferences may evolve over time
